1. If you are an individual donor, you affirm that: (i) you are at least 18 years old, a U.S. citizen or lawful permanent resident; (ii) you have not knowingly or willfully accepted funds, directly or indirectly, of more than $10,000 from one or more prohibited sources* within two years of making this contribution; and (iii) your contribution is made from your personal funds and that you will not be reimbursed for the contribution from another person or entity.
   
2. If you are an entity donor, you affirm the entity: (i) is organized pursuant to U.S. or state law, the U.S. is its principal place of business, and is wholly or majority owned by U.S. citizens or lawful permanent residents; and (ii) has not knowingly or willfully accepted funds, directly or indirectly, of more than $10,000 from one or more prohibited sources* within four years of making this contribution. 
   

“Prohibited sources” means “contributions from or expenditures by a foreign national made with the intent to use such funds to influence an election on a ballot measure.”
